Refer a Patient for Behavioral Health Services

We welcome referrals from healthcare providers, hospitals, schools, case managers, social service agencies, community organizations, and other professionals seeking behavioral health services for their patients or clients.

Our behavioral health services provide compassionate, confidential, patient-centered care. Services may include comprehensive psychiatric evaluations, medication management, treatment of depression and anxiety, ADHD evaluation and management, mood disorders, trauma-related conditions, substance use concerns, and ongoing psychiatric follow-up.

How to Refer a Patient

To submit a referral, please complete our Behavioral Health Referral Form and provide relevant clinical documentation, including current diagnoses, medication lists, recent behavioral health assessments, laboratory results when applicable, and insurance information.

Once the referral is received, our team will review the information and contact the patient or authorized representative regarding scheduling and next steps.

For urgent or emergency psychiatric concerns: If a patient is experiencing an immediate safety concern, suicidal or homicidal thoughts with imminent risk, or another psychiatric emergency, call 911 or direct the patient to the nearest emergency department. Referral submissions are not monitored as an emergency response service.
